Patient forms are conveniently available online for completion prior to your appointment. If you prefer to complete them in person, please arrive 10 minutes before your scheduled appointment time. For families with multiple children, we recommend arriving 15-20 minutes early.
Please note that only parents or legal guardians are authorized to sign treatment consents and health questionnaires. For initial appointments, the parent or legal guardian must be present with the patient. For subsequent appointments, a relative or step-parent may accompany the patient, but they will not be able to sign legal consents.
We understand that unforeseen circumstances can arise. To accommodate minor delays, we offer a 10-minute grace period. While we will make every effort to accommodate late arrivals beyond this period, rescheduling may be necessary depending on availability.
To ensure efficient scheduling and respect for all patients, we have a "no call, no show" policy. Repeated missed appointments may result in the inability to schedule future appointments.
If you need to cancel or reschedule your appointment, we kindly request that you provide us with at least 48 hours' notice.
